BASIS-24®
2019 – 2023

Timberline Knolls utilizes the 24-item Behavior and Symptom Identification Scale, BASIS-24®, a leading behavioral health assessment tool. The BASIS-24® identifies a wide range of symptoms and problems that occur across the diagnostic spectrum. Grounded in the latest scientific methods of survey development and validation, the BASIS-24® underwent extensive field testing as part of a multiyear research and development process. BASIS-24® is validated in both Spanish and English.  The 24 questions are scored using a weighted average algorithm that gives an overall score and scores for six subscales for the following domains of psychiatric and substance abuse symptoms and functioning: Depression and Functioning, Relationships, Self-Harm, Emotional Lability, Psychosis, and Substance Abuse.

The BASIS-24® is designed for use by mental health providers, researchers, purchasers of mental health services, accreditation agencies, and internal quality assurance departments to measure the change in consumer self-reported symptom and problem difficulty over the course of treatment. The outcome measure is administered at admission, mid-point, and discharge. Participation is voluntary and encouraged.


The transition into PHP from Residential was the most scary but wonderful thing that happened to me during my TK experience. It was hard to leave 24/7 care, and I was nervous about changing the atmosphere around me but once I stepped foot in that building I had a certain feeling of calm. I was approached by all different people introducing me to the new world I was now a part of.  My housemate gave me the best welcome and helped me get settled in.  I found that the groups in PHP gave me a new and different experience in treatment, and I was able to look at things from an entirely point of view.  There were groups I attended that didn’t happen on lodge, and the staff was there to help support me, yet give me the gentle nudge I needed to help myself and figure out how I was going to be able to survive in my world at home without all the people I have been getting use to as backups for me. I can’t say it was a seamless transition but I learned that nothing is a straight line and you cannot know until you try. Being able to utilize the step down process gave me the skills I needed to succeed in real life and flourish in whatever I choose to do.
